To provide some sort of context to the custom attribute see the image below.
This inline component has a graphic element that extends beyond the body of the component. The body is the actual portion that needs to break the line. If the custom attribute is set to TRUE then the body of the component will be the break point (As defined by the Connect Points). When the custom attribute is set to FALSE then the cell extents are used to govern the break points.

This is typically seen with valves that have built in handles and flow elements that have additional graphics that indicate flow direction.
