The example I gave is one of the cases that could show duplicates in Line List - Pipe Run. As well "duplicates" are shown when To/From arrow are used. In case pipe run is linked to several drawing using To/From arrows this will be reflected in Data Manager view.
Not sure if this will help but there is a "Purge To/From Arrows " in P&ID Tools. Please refer to the Help file for more details about its functionality.
As for the crash it could be that it is related to version of AutoCAD you are using. AutoPlant 08.11.09.140 supports only AutoCAD 2012.
As this is a specific case maybe you could submit a service request for issue.
